The entire application must be completed to be considered for an award, with each attachment clearly labeled. At the end of the application are appendices that will assist with certain sections. These appendices are not meant to be completed or submitted to OKI; the appendices are provided to assist with the preparation of the application.
If application includes vehicle purchase, a public notice (refer to Attachment N and Appendix 2-3) of your agency’s application must be completed by 1/27/17 (3 week extension)
Applications must be submitted to OKI by 02/08/17 (2.5 week extension).
The Application Certificate page must be signed in order for an application to be considered.
